PROMISING RESULTS ON BREAST CANCER STUDY WITH HIFU

Dr. Alessandro Napoli, M.D., Ph.D., assistant professor of radiology at Sapienza University in Rome, presented the results of his study involving 12 patients with invasive ductal breast cancer. All patients were treated with FUS, and post-treatment, histological evaluation confirmed the absence of residual disease in the treatment area in 10 patients. Though this study is part of early stage research, Dr. Napoli is hopeful that focused ultrasound will one day be a treatment option for women with breast cancer.

The Doctors at Churchill Hospital in Oxford (Radcliffe Hospitals NHS) where Professor Sir Peter Morris, President of Royal College of Surgeons say Breast cancer patients’ lumps could instead be “zapped” with ultrasound, sparing them the trauma of breast removal.

What is Focused Ultrasound ?

Focused ultrasound therapy uses ultrasound waves to guide the application of focused, high-intensity ultrasound waves through the skin down to the target tissue. The high-intensity sound waves are focused on the individual targets much like using a magnifying glass to focus the sun’s rays – only the focal point becomes hot enough to destroy the unwanted or problematic tissue. The Ultrasound gives the treating physician a three-dimensional view of the tumor in the breast, etc. to be treated and provides real-time feedback about the temperature and tissue changes that occur as each tissue is ablated. It requires no incision in an outpatient setting and allowing a rapid return to family or work.

HIFU treatment for Breast Cancer with NUTAS

A total of 106 patients with biopsy-proven breast cancer underwent extracorporeal HIFU ablation. Of 22 patients received breast conservation treatment with HIFU in Beijing center, in combination with adjuvant chemotherapy, postoperative auxiliary node dissection or/and postoperative radiation therapy. The mean follow-up time for patients with breast conservation treatment in our center was 22 months (range 10–36 months). Total resorption of ablated tumor was found half of them within 1–2 years after HIFU treatment, which mainly correlated with the increase of follow-up time and tumor size. All patients are still alive in the follow-up time and all but one is disease-free. One case presented with local recurrence after 18 months in the targeted region and this patient was subsequently treated by the modified radical mastectomy.

WHAT IS HIFU

HIFU (sometimes FUS or HIFUS) is the abbreviations of High-Intensity Focused Ultrasound (focused Ultrasound of high intensity). It consists of focusing the energy of a beam of ultrasound of high frequency and discharge energy in a very small volume. Applying in weaves and when it reaches sufficient energy in the center takes place cellulardestruction (coagulativa necrosis) of the cells of the objective volume by elevation of temperature among 55-100ºC (thermal ablation). With the particularitity of which the adjacent weaves are not affected. The cellular destruction takes place very selectively and with oval form, in the transducers more advanced the volume of the center arrives to be from 2x2x15 mm3.

The objective of systems HIFU is to lift the temperature of the point to treat over 55-60 ºC during 1 s or more time, to produce coagulativa necrosis and the immediate cellular death. With the sufficient compression and expansion of the weave air bubbles are obtained. These air bubbles interact with the ultrasound producing a liquidflow that can damage solid surfaces.

The therapy with HIFU can produce apoptosis by direct injury of DNA, that in fact is the primary mechanism of cellular death by hyperthermia.
The effects nonlinear of ultrasound must to the modification of the wave of ultrasounds within the weave by their heterogeneity and contributes to the temperature rise.

History

The origin of the HIFU is not recent, goes back to years 50 in the USA, where the first investigations on the part of Lindstrom and the Fry brothers in animal brains began. He was later when some Chinese disciples, of the American investigators were placed to the front of the investigations of this technology until arriving, at the beginning of years 90, to the clinical use in real patients.

Types

2 types exist basically the guidance system: – HIFU Guided by magnetic resonance (MRgFUS) – HIFU guided by ultrasounds or ecógrafo (USgFUS)

HIFU guided by resonance allows one more a clearer image of the zone to deals with the patient but it does not allow real visualization in time. And so possible the movements of the patient could harm the treatment. One monitors the changes of temperature of weaves to determine the HIFU effects. The treatment transducer has little mobility due to its integration with the machine of magnetic resonance. A room equipped especially for magnetic resonances is needed.

HIFU guided by ultrasounds or ecógrafo allows an exploration in real time and it is observed the changes in weavesby means of changes in the gray scale of the ecográfica image. The installation of the USgFUS machine is the one of a conventional room.

HIFU takes being used in clinical treatment from 1997. One is used in China, South Korea, Japan, Malaysia, Russia, Canada, South America, the USA, the United Kingdom, Poland, Austria, Germany, France, Saudi Arabia, Lebanon, Syria, South Africa, Turkey, Australia, Portugal, Italy and Spain. One has treated anywhere in the world 60,000 patients by means of técnia HIFU.

Disadvantages

Contraindications

The limitations of HIFU come given by the own nature from the ultrasound in combination by the average ones where it propagates. The ultrasound bounces when running into with gas, reason why it is not possible to treat bodies that gas of natural form contains. And the bones also produce a high attenuation to the ultrasound.

Ablatherm Of company EDAP settled down in Lyons, France. Guidance by ultrasounds and is only applicable to prostate neoplasia. It has 184 centers in all the world. In 2006 it treated 3500 patients. The treatment is minimumly invasive since it is had to introduce a sounding to endorectar to realize the treatment.

ExAblade2000 Of company INSIGHTEC settled down in Israel. Guidance by magnetic resonance of General Electrics.Only indicated for gynecological benign tumors. Power and average precision, the movement of the transducer is limited. Guidance is not in real time. 18 centers in the USA and 20 exist outside.

There are others like Sonar Blate 500 of USHIFU of the USA, similar to Ablatherm. Besides Shanghai Aishen, Medical China Technologies, and Minayang are competing with a less powerful and versatile product than the one of Haifu ChongQing. One knows that Siemens is working together Haifu to develop a model of HIFU guided by magneticresonance that improves to the benefits of the INSIGHTEC model.

Also llamdo Liposonix of a company of the USA of the same name for the esthetic treatments of thinning exists a model.

Conclusions

HIFU is a new therapeutic strategy with a recognized physical base, profile of low toxicity and concrete indications on the basis of the works published by the institutions with experience, with the described advantages and limitations, and whose paper in our country has to be defined in the multidisciplinary context of the implied specialists.

With the introduction of the HIFU we are initiating a process that, without a doubt, breaks some therapeutic schemes and modifies paradigms and, therefore, like all change, can create inconveniences and certain reluctance in some professionals of the medical group.

We are convinced that technology HIFU will extend quickly by Europe.

The American doctors use already it habitually for benign gynecological pathology and they themselves recognize, in one recent scientific publication of the month of January of 2008, the weight that will in the very near future have the HIFU in the treatment of diverse types of solid tumors.
